At CareAparent you will work with other passionate individuals focused on giving the best care possible for our clients. The Registered Nurse Case Manager (RNCM) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and supervising HHA's and CNAs. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Provide compassionate care to seniors who need it most. Your services help clients stay in their home, maintain a comfortable lifestyle, and live happier lives.

Pay range: $39.00 - $45.00/hour

Duties & Responsibilities:

  • Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and Recertifications
  • Work within primary zone (local area), covering other zones as needed
  • Develop a plan of care based on physician orders and initial client assessment and continue to monitor, revise, and update based on ongoing assessments or changes of condition.
  • Facilitate referrals to interdisciplinary teams (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention).
  • Perform skilled nursing tasks as assigned in coordination with the Clinical Manager and CareAparent scheduling.
  • Oversee and supervise Licensed Practical Nurse (LPN) visits and ensure plan of care is being followed.
  • Provides clinical oversight, delegation, and leadership to licensed clinicians and unlicensed care team members.
  • Prepare visit/shift reports, patient records summaries, and lead care team case conferences with the care team.
  • Coordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followed.
  • Report on changes in client condition to physicians and Clinical Manager.
  • Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms.


Qualifications:

  • Current Minnesota RN license
  • Previous experience with seniors and client assessments
  • Excellent customer relations and interpersonal skills
  • Excellent verbal/written communication skills
  • Ok with travel to and from client homes
  • Ability to work on call for rotating weekends, and holidays
  • Possess a valid driver's license and insured personal vehicle
