We are seeking a Rehabilitation Coordinator to join The Center for Rehabilitation at ChristianaCare. In this role, you will be responsible and accountable for overseeing the Uniform Data System/Preferred Payment System (UDS/PPS) process, ensuring timely and accurate submissions to regulatory agencies to support appropriate reimbursement. Principal Duties & Responsibilities Coordinate the pre-admission process for The Center for Rehabilitation. Document accurately in the electronic medical record and admissions-support platforms to support the admissions workflow. Prepare and provide summaries of admission and pre-admission documentation for review with the Medical Director. Serve as a professional liaison between The Center for Rehabilitation and referral sources, including external caregivers and partners. Provide education to patients and families about the Center’s services, setting expectations for acute rehab to support positive outcomes and an excellent patient experience. Educate referral sources about the acute rehab admissions process and assist them in identifying appropriate candidates for admission. Communicate effectively with patients, physicians, payers, employers, providers, vendors, and other healthcare team members. Serve as an expert resource to the healthcare team, monitoring documentation and ensuring sufficient information is gathered for referral decisions. Assist with data collection and the implementation of action plans for CMS, JCAHO, AHA quality initiatives, and ChristianaCare performance goals. Collaborate with hospital performance improvement efforts, providing additional documentation as needed. This position will require travel between ChristianaCare and referral locations Education & Experience Requirements Master’s or higher degree in Physical, Occupational, or Speech Therapy; or Bachelor’s degree or higher in nursing or social work Minimum of three (3) years clinical rehabilitation experience and/or insurance rehabilitation experience. Annual Compensation Range $79,497.60 - $127,212.80 This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting.
